POLICIES AND WAIVER
ENROLLMENT: Enrollment in a class or classes requires full payment of a
non-refundable Registration Fee. Only the Initial
Tuition fee for enrollment will be prorated, due to
missed classes. DMDS reserves the right to cancel a
class due to insufficient enrollment and will offer
an alternate class to attend.
TUITION:
Tuition is paid monthly to hold a student�s place in
class or classes, and must be paid regardless of the
student�s attendance each month. Some months will
have five weeks of class while others will have
less; however, monthly tuition will remain the same.
Tuition is due
by the first of the month, and considered past due
after the 10th, at that time a $25.00
late fee is added to your account. If tuition
becomes two months delinquent students are subject
to removal from classes.
MISSED CLASSES: We do not offer make-up classes.
NO reductions or refunds are given for missed
classes.
PAYMENTS:
We suggest that you have a credit card on
file to be debited monthly for tuition. Other fees,
such as costumes or Spirit wear will be charged to
your card on file, unless notice is given with
another form of payment or non-participation of
Recital.
HOLIDAYS: Please remember, there is NO prorating
for the Holiday Months. Dates are subject to
change.
LOST AND FOUND: Items lost at the studio are kept
for 30 days only. Please check the office regularly
as items are donated monthly. DMDS will not be held
responsible for lost or stolen items.
SAFETY: I hereby release and hold harmless Dance
Machines Dance Studio, Inc., its owners and all
employees from all liabilities, suits, claims and/or
demands of any kind or nature, legal or financial,
whether caused in any way by negligence or
participation in any activities. The student
registered does voluntarily participate in Dance
Machine Dance Studio Inc., and I understand that
certain risks are inherent to and from participation
and involvement in various activities. Parent or
guardian must be responsible for Medical Insurance
on your student.
DRESS CODE: Dress code is given by the teacher of each class. DMDS
strongly suggest that dress code is adhered to.
This is for your dancers benefit. Without proper
clothes it is difficult to critique movement and
style.
WITHDRAWALS:
Students are registered continually on a month to
month basis, unless we are given a 30 day notice.
If notice is not received, you will be charged your
monthly tuition rate.

GIVE WRITTEN NOTICE TO AVOID ADDITIONAL TUITION***
RECITAL CLASS COSTUMES: Each student is measured in
November for their costume. All costumes are
ordered in December to insure delivery before end of
April/first of May, for necessary fittings and
alterations. Costumes ordered after December will
incur a late fee. (Costume Price includes: Costume,
1 pair of tights or socks, accessories)
NO REFUNDS will be
given on Costumes, and costumes must be picked up
within 30 days or they will become property of Dance
Machines.
RECITAL AWARDS:
If not picked up at Recital, they will be held at
the studio for 30 days, after that they will become property of Dance Machines.
PHOTO RELEASE:
I am aware
that photographs and videos may be taken of my
child/children by DMDS staff members. I give my
permission to DMDS to use such photographs and
videos for promotional and advertising
purposes.
I
